This study developed a Faculty Performance Evaluation System (FPES) of the Don Eulogio De Guzman Memorial National High School (DEGMNHS) It specifically addressed the following objectives: To develop an instrument to be used by the students to evaluate the performance of the faculty members of DEGMNHS, To design a FPES for the DEGMNHS, and To determine the level of usability of the system. The researcher found out that: there is no existing process of faculty performance evaluation of DEGMNHS but have an Individual Performance Commitment and Review Form for self¬ performance. The instrument developed is appropriate in the development of the FPES of the DEGMNHS since it has been validated with the rating of very highly valid. The FPES of DEGMNHS was developed using SCRUM model. The usability of FPES has been evaluated with the score of 74.93 which means the FPES is usable with the used of the SUS questionnaire.
