Specifically, it sought answers to the following objectives: to determine the ICT Profile of the Balaoan District Hospital along: hardware, software, peopleware, and network; to assess the status of existing system in the Balaoan District Hospital in terms of: performance, information, economics, control and security, efficiency, and services; to develop a patient information system for Balaoan District Hospital; to assess the usability of the developed system in terms of: affect, efficiency, learnability, helpfulness, and control. The descriptive and developmental research designs with the survey checklist and questionnaire as gathering tools were used in the study. A total of one-hundred-five (105) respondents from the employees and patients of the Balaoan District Hospital located at Brgy. Bungol, Balaoan, La Union was requested to participate in the survey floated by the researchers. Among these respondents, the Chief of Hospital along with eight (8) Administrative Personnel were involved in the determination of ICT Profile of the Balaoan District Hospital as well as in the assessment of the usability of the developed system. On the other hand, ninety-six (96) patients were also requested to participate in the assessment of the status of the existing system of the hospital. Data gathered were analyzed by way of frequency count and mean. In the determination of ICT Profile of the Balaoan District Hospital, results depicts that only hardware and software are present while the peopleware and network components seem to be insufficient in accommodating the deployment of the system. In the assessment of the status of existing system in the Balaoan District Hospital, results entail that the all components of the PIECES Framework are found to be constraint. The Balaoan District Hospital Patient Information System (BLDH-PIS) was developed using Rapid Application Development model and was assessed as usable by the respondents.
