The study aimed to develop the Barangay Health Center Record Management System of Rosario, La Union. Specifically, this study aimed to: (1) identify the features of the Barangay Health Center Record Management System, (2) design and build the system using Rapid Application Development (RAD) Model, and (3) assess the software quality based on the ISO 9126 Software Quality Criteria with the six (6) evaluation criteria: functionality, reliability, usability, efficiency, maintainability, and portability. A descriptive research design was implemented with the aid of interviews and ISO 9126 software quality criteria as a data gathering instrument, and a developmental research design was implemented for the development of the system using the RAD model. Features of the record management system include managing records of Maternal Records, Child Records, Family Planning Records, and generating and printing reports. The system was evaluated as usable with a weighted mean of 4.53.
